First of all, thanks for your interesting in the plug-in.

Generally, you can see the field name of email address and name by...

1. Let auto-responder generate an opt-in form code for you.
2. Find for &lt;code&gt;&#60;input type="text" ...&lt;/code&gt; This will represent the textbox.
3. When you found it, just look before it and you will see what is this textbox for.
4. If it's emailaddress or name then look for "id" or "name" within the &lt;code&gt;&#60;input ...&lt;/code&gt; tag.

Such as 
&lt;code&gt;
&#60;tr&#62;&#60;td&#62;Your Name : &#60;/td&#62;&#60;td&#62;&#60;input type="text" id="name1"&#62;&#60;/td&#62;&#60;/tr&#62;
&lt;/code&gt;
You will see this is for "Name" and its id is "name1" and do this the same with email address.

*Note that, in the next update of the upgraded version, I'll include automatic field extraction. You just paste the code generated by your auto-responder.
